Position Overview: Patient Care Coordinator – Enteral Nutrition

As a Patient Care Coordinator in our Enteral Nutrition division, you will serve as a critical point of contact for patients, caregivers, healthcare providers, and internal departments. Your primary responsibility will be to ensure seamless coordination of care for patients requiring enteral (tube) feeding support. Key Responsibilities

As a Patient Care Coordinator, your daily responsibilities will include but are not limited to:


  • Patient Communication: Professionally answer incoming calls from patients, caregivers, branch locations, and patient reference sources regarding nutrition therapy request status, ensuring all inquiries are handled with empathy and efficiency.

  • Call Management: Triage calls to appropriate internal departments based on the nature of the inquiry, ensuring patients are quickly connected to the right resources for their needs.

  • Documentation Accuracy: Verify patient demographics and insurance information to guarantee accurate shipment of nutritional products and maintain detailed records in our electronic systems.

  • Order Processing: Handle monthly patient refill requests, ensuring all documentation is complete and accurate before orders are processed for shipment.

  • New Patient Integration: Work collaboratively with the patient admissions department to assist in welcoming new patients to our services, providing them with essential information and setting the foundation for positive care experiences.

  • Electronic Health Records: Update and maintain electronic clinical records with precision, ensuring all patient information is current, accurate, and compliant with regulatory standards.

  • Quality Assurance: Review documentation for accuracy and completeness, identifying and resolving any discrepancies to ensure optimal patient safety and care continuity.

Required Qualifications

To succeed in this role, candidates must meet the following requirements:


  • Education: Bachelor's degree from an accredited institution is required.

  • Customer Service Experience: Minimum of one year experience working in a customer service environment, demonstrating strong communication skills and a patient-first approach.

  • Data Entry Skills: Proficiency in data entry with a strong attention to detail and accuracy.

  • Technical Skills: Experience working in Microsoft Office applications, specifically:

    • Excel: Ability to work with spreadsheets, data organization, and basic formulas

Preferred Qualifications

  • Durable Medical Equipment (DME): Experience with home infusion therapy or durable medical equipment (DME) is highly desirable.

Compensation and Benefits
