About the position
The RN CAR-T Navigator is a registered nurse with oncology-specific clinical knowledge who offers individualized assistance and care coordination to patients receiving CAR-T therapy within the organization.
- Responsibilities
- Serve as a liaison for the patient and family with their oncologist and other healthcare providers to ensure effective communication and collaboration
- Monitor and provide patient symptom management
- Manage high risk, complex patient care with the goal of minimizing emergency department and inpatient readmission
- Assess barriers to care to address patient, caregiver, or family needs to achieve optimal patient outcome
- Provide patient-centered individualized ongoing education, resources, and referrals to internal and external resources to patient and caregivers
- Complete requested clinical documentation as needed
- Promote a patient and family-centered care environment for ethical decision making and advocacy for patients with cancer
- Establish and maintain professional role boundaries with patients, caregivers, and the multidisciplinary care team in collaboration with clinic and senior leadership
- In collaboration with other members of the healthcare team, build partnerships with local agencies and groups that may assist with cancer patient care, support, or educational needs
- Use an ethical framework regarding patient care to assist patients with cancer with issues related to treatment goals, advance directives, palliative care, and end-of-life concerns
- Facilitate communication among members of the multidisciplinary cancer care team to prevent fragmented or delayed care that could adversely affect patient outcomes
- Coordinate with the RN Quality Coordinator to provide documentation for the CAR-T program
- Attend regular meetings with CAR-T program development team
- Requirements
- Registered Nurse, licensed in the State of Employment
- 2 years oncology nursing experience required (preferred knowledge about the transplant/cellular therapy process)
- Essential competencies and KSAs targeted to successfully performing in role: Analysis & Critical Thinking Decision making and problem-solving ability to resolve complex clinical, systems, and personnel problems and issues
- Strong interpersonal skills to include effective verbal and written communication
- Strong Leadership skills and organizational capability
- Solid time management with the ability to prioritize multiple tasks
- Ability to collaborate across various levels of management, departments, and teams
- Comfortable negotiating problems and exploring solutions with physician population
- Nice-to-haves
- BSN (Preferred)
- OCN or BMTCN certification preferred
- Current Basic Life Support Certification
