Care Manager RN - Part-time - REMOTE
Posted 2026-05-06The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Communicates on a daily basis with medical staff, hospital personnel and outside resources to ensure the appropriate utilization of resources according to clinical and financial standards. Monitors the medical record for variances to clinical and financial standards and intervenes to achieve the optimum outcome for the patient and the hospital.

Required Qualifications:
- Associate's Degree in Nursing or a nursing diploma upon hire.
- Upon hire: California Registered Nurse License
- 2 years of acute care experience in a Medical Surgical or Inpatient setting.
- 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.
Preferred Qualifications:
- Bachelor's Degree in Nursing or higher within 3 years of hire.
- CCM or ACM
