</div><div>What You'll Do<br><br>
</div><div>· Provide skilled nursing care during home visits<br><br>
</div><div>· Assess patient needs and help create individualized plans of care<br><br>
</div><div>· Administer medications, treatments, and wound care as ordered<br><br>
</div><div>· Monitor patient progress and communicate changes with the care team<br><br>
</div><div>· Educate patients and families with patience, empathy, and encouragement<br><br>
</div><div>· Support patients in maintaining comfort, independence, and quality of life<br><br>
</div><div>· Complete documentation accurately and in a timely manner<br><br>
</div><div>· Collaborate with physicians, therapists, aides, and other team members to ensure coordinated care<br><br>
</div><div>What We're Looking For<br><br>
</div><div>· Current and active Registered Nurse (RN) license in Indiana<br><br>
</div><div>· Minimum of 1 year of nursing experience<br><br>
</div><div>· Home health experience required<br><br>
</div><div>· Strong clinical judgment and assessment skills<br><br>
</div><div>· A caring, dependable, and patient-centered approach<br><br>
</div><div>· Excellent communication and time-management skills<br><br>
</div><div>· Current CPR certification<br><br>
</div><div>· Valid driver’s license, reliable transportation, and auto insurance<br><br>
