Hourly compensation between $21.80 - $24.20, PLUS an annual performance-based, discretionary incentive.

What You Will Do - Essential Functions

The Family Support Coordinator provides proactive outreach, assessment, education, and care coordination support to families of children in the Autism Resource Program by connecting them to appropriate services, navigating benefits, and ensuring timely access to care and resources.

Family Engagement & Needs Assessment

Conduct outreach and screening calls to assess family needs, identify barriers, and evaluate gaps in care.

Build relationships with families through education, support, and ongoing engagement to ensure effective program participation.

Care Coordination & Resource Navigation

Facilitate referrals to clinical, community, and government resources and coordinate with care managers, providers, and internal teams.

Act as a central point of contact to triage cases, ensure documentation is complete, and support timely access to services.

Program Education & Administrative Support

Educate families on benefits, coverage, behavioral health services, and program requirements.

Provide administrative and operational support including documentation, communication, and compliance with organizational and regulatory standards.

Who You Are

Required Qualifications

Bachelor’s degree in Social Work, Psychology or related healthcare field and three (3) or more years of experience in a health care, managed care and/or customer service setting

OR Associates degree and 4+ years of experience

OR high school diploma and 5+ years of experience in a health care, managed care or customer service setting

Strong computer and keyboarding skills (MS Outlook, MS Word, MS Excel, Adobe Acrobat, and data processing), including the ability to document while simultaneously while taking information over the phone

Excellent written and verbal communication, interpersonal, and organizational skills

Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Working Conditions:

High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.

Frequent use of computer and phone systems

Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.

Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.

A quiet workspace with minimal background noise for calls.

This role requires working from a fixed, designated workspace in your home and does not allow flexibility in work locations even within the home.

A hardwired internet connection is required along with ability to sit for extended periods using company-issued wired headsets and desktop equipment.
