- Job Description:
- Collaborates with patients and healthcare teams to navigate patient needs through the healthcare continuum, including appointment coordination, record requests, insurance concerns, education, and communication between the physician office and the patient.
- Maintains relationships with physicians and clinical leadership to assist consumers, including travel for education, collaboration, and expanding knowledge of network facilities and offerings.
- Assists leadership with program development, functioning as a liaison with stakeholder teams, and providing data and departmental information.
- Completes accurate data collection as directed by the supervisor and submits monthly reports in a timely and efficient manner.
- Supports program awareness within the physician community and the broader community, both in person and virtually, in collaboration with leadership and CHN teams.
Education:
- Bachelor's of Science degree in Nursing [Required]
- Master's of Nursing [Preferred]
Work Experience:
- 1+ years of nursing in outpatient setting [Required]
- 2+ years of hospital care coordination experience [Preferred]
Licenses and Certifications:
- Registered Nurse (RN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:
$68,132.50 - $119,520.35
