Hospice of Lenawee is hiring a compassionate Registered Nurse (RN) Case Manager to provide high-quality, patient-centered hospice care in patients’ homes throughout our 35-mile service area which includes Lenawee County and surrounding areas.

Schedule


  • Full-time (40 hours/week)
  • Day shift
  • Limited weekend rotation
  • Holiday rotation (3–4 per year)

Requirements


RN Case Manager Responsibilities


  • Provide skilled nursing care to hospice patients in their homes
  • Manage a caseload of patients and families
  • Develop and coordinate individualized care plans
  • Monitor and manage pain and symptoms
  • Educate and support patients and caregivers
  • Collaborate with physicians and interdisciplinary team members
  • Document care in accordance with regulatory standards

Qualifications


Required:


  • Active Michigan RN license
  • Graduate of an accredited nursing program
  • CPR certification
  • Valid driver’s license and insured vehicle
  • Ability to travel locally within Lenawee County and surrounding areas within our 35-mile service area

Preferred:


  • BSN
  • Experience in hospice, home health, oncology, or case management
  • Experience providing end-of-life care
