</div><div><strong>Responsibilities:</strong></div><ul>
<li>Conduct comprehensive assessments of patients transitioning from hospital to home care.</li>
<li>Develop and implement personalized care plans in collaboration with healthcare teams.</li>
<li>Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.</li>
<li>Educate patients and families on post-discharge care and available resources.</li>
<li>Facilitate communication between patients, caregivers, and healthcare providers.</li>
<li>Document all patient interactions and care plans in the electronic health record (EHR).</li>
<li>Participate in quality improvement initiatives to enhance care delivery.</li>
<li>Stay updated on best practices in transitions of care and healthcare regulations.</li>
</ul><div><strong>Requirements:</strong></div><ul>
<li>Active RN license in the state of Georgia.</li>
<li>Minimum of 2 years of nursing experience, preferably in acute care or case management.</li>
<li>Strong knowledge of transitions of care processes and patient-centered care.</li>
<li>Excellent communication and interpersonal skills for patient engagement.</li>
<li>Proficiency in electronic health record (EHR) systems.</li>
<li>Ability to work independently and manage time effectively in a remote setting.</li>
<li>Compassionate demeanor with a commitment to improving patient health outcomes.</li>
<li>Certification in case management (CCM) or transitions of care preferred.</li>
