- Open to qualified candidates nationwide Key Responsibilities
- Review remote patient monitoring data (blood pressure, glucose, weight, vitals) and identify trends or concerns
- Conduct monthly and ongoing patient outreach to support care plans and medication adherence
- Complete a minimum of 15 unique patients per day receiving billable care or at least 50 outbound calls daily
- Coordinate with primary care providers and specialists as needed
- Accurately document patient conversations and care coordination in the EHR
- Provide education, encouragement, and clear guidance to patients managing chronic conditions
- Participate in weekly team meetings, weekly one-on-ones, and monthly office touch-base calls Work Expectations (Non-Negotiable)
- Maintain agreed-upon working hours and notify management in advance of any schedule changes
- Expect to spend approximately 50% of your shift on live patient calls
- Calls, documentation, and activity are monitored for quality and training purposes
- A paid 15-minute daily morning routine is required for planning, reviewing RPM alerts, and follow-ups
- Billable care requires an actual patient conversation; no more than 10 minutes may be documented without patient contact
- Performance and workload are reviewed weekly Qualifications
- Active or recently retired MA, LPN, or RN (license preferred; requirements vary by state)
- Minimum 3 years of experience in primary care, home health, care coordination, or chronic disease management
- Strong communication skills with a calm, patient-first approach
- Comfortable with technology, EHRs, and remote monitoring platforms
