Job Description Summary

We are seeking a highly skilled and empathetic Advanced Practice Registered Nurse (APRN) to join our telehealth team. This specialized role focuses on providing comprehensive memory assessment and care through a virtual platform. You will be responsible for conducting cognitive screenings managing the symptoms of patients living with dementia.

A portion of this role involves managing the complex behavioral and psychological symptoms of dementia (BPSD), ensuring that both patients and their caregivers have a clear, medically sound path forward from the comfort of their homes.‎
How will you make an impact & Requirements

Key Responsibilities

Virtual Assessments: Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations.

Cognitive Screening: Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) to determine the stage and type of cognitive impairment.

Psychiatric Management: Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions.

Caregiver Support: Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques.

Documentation: Maintain electronic health records (EHR) in compliance with Florida state regulations and HIPAA standards for telehealth.

Qualifications & Requirements

Licensure: Must hold a current, unencumbered Florida APRN license with prescriptive authority.

Education: Master’s or Doctoral degree in Nursing (MSN or DNP).

Clinical Experience: Minimum of 2–3 years of experience specifically in geriatrics, neurology, or a dedicated memory care setting preferred.

Telehealth Proficiency: Experience conducting video-based clinical visits and comfort troubleshooting basic technology issues with elderly patients and their families.

Specialized Knowledge: Working knowledge of the CMS Cognitive Assessment & Care Plan requirements and the nuances of dementia-related behavioral health.

Technical Requirements
