Position Overview


We are looking for a motivated and detail-oriented Case Management Specialist to join our remote team. In this role, you will serve as a critical link between our members, healthcare providers, and community organizations. You will leverage your analytical skills and clinical knowledge to coordinate comprehensive care plans that address the full continuum of our members' healthcare and social determinant needs.


This position is ideal for professionals who are passionate about healthcare, possess strong communication skills, and want to make a meaningful impact in the lives of dual-eligible beneficiaries (those enrolled in both Medicare and Medicaid). You will work closely with case managers, clinical directors, and other health programs to overcome barriers and achieve optimal health outcomes for our members.

Key Responsibilities



  • Member Assessment: Conduct comprehensive assessments of referred members using care management tools and data review. Evaluate member needs, eligibility, and develop appropriate approaches to case resolution by reviewing benefit plans and available internal and external programs.

  • Risk Identification: Identify high-risk factors and service needs that may impact member outcomes. Collaborate with clinical case management or emergency intervention teams as appropriate to ensure comprehensive care coordination.

  • Care Plan Coordination: Organize and implement assigned care plan activities while monitoring progress toward goals. Ensure all interventions align with best practices and member-specific needs.

  • Quality of Care Enhancement: Work collaboratively with case managers, supervisors, Clinical Directors, and other health programs to overcome barriers to meeting objectives. Present cases at case conferences to obtain multidisciplinary review for achieving optimal outcomes.

  • Care Issue Resolution: Identify and escalate quality of care issues through established channels. Utilize negotiation skills to secure appropriate decisions and services necessary to meet member benefits and healthcare needs.

  • Member Empowerment: Use influencing and persuasive communication skills to ensure maximum member engagement and promote lifestyle/behavior changes for achieving optimal health levels. Provide coaching, information, and support to empower members in making informed clinical and healthy lifestyle choices.

  • Healthcare Navigation: Assist members in effectively and knowledgeably navigating the healthcare system by facilitating communication between members and their providers.

  • Documentation and Compliance: Maintain accurate documentation of all care coordination activities using case management and quality management processes in compliance with regulatory, licensure, and company policies and procedures.

Essential Qualifications



  • Experience: Minimum of 3+ years of experience in a health-related field

  • Education: Bachelor's Degree or equivalent experience required

  • Technical Skills: Proficiency with care management tools, data analysis, and electronic health record systems

  • Communication: Excellent verbal and written communication skills with the ability to interact professionally with diverse populations

  • Critical Thinking: Strong analytical and problem-solving abilities to assess complex situations and develop appropriate solutions

  • Organization: Exceptional organizational skills with the ability to manage multiple cases and priorities effectively

  • Remote Work: Must have a dedicated home office space with reliable high-speed internet connectivity

Preferred Qualifications



  • Professional certifications: CRC (Certified Rehabilitation Counselor), CDMS (Certified Disability Management Specialist), CRRN (Certified Rehabilitation Registered Nurse), COHN (Certified Occupational Health Nurse), or CCM (Certified Case Manager)

  • Experience with Medicare and Medicaid programs

  • Background in managed care environments

  • Experience working with geriatric populations, individuals with special needs, social health concerns, and disabled populations

  • Knowledge of assessment tools, screenings, and care planning methodologies

  • Bilingual proficiency (English/Spanish or English/Creole) is highly desirable

  • Experience with DSNP (Dual Eligible Special Needs Plans) programs

Work Schedule


This is a full-time remote position with standard business hours: Monday through Friday, 8:00 AM - 5:00 PM CST. As a member of our team, you'll enjoy the flexibility of working from home while still being connected to a supportive team environment.

Compensation and Benefits
