Key Responsibilities:
Collaboratively develop an individualized care plan, behavior goals and learning objectives with patients of chronic conditions, especially diabetes and hypertension. Provide individual case management to patients and keep documents under HIPAA compliance.
Use the established Standards of Practice and Professional Performance (SOP/SOPP) in diabetes care, hypertension management and relevant chronic disease management and evidence-based practice guidelines to determine the educational interventions for each individual.
Collaborate with primary care physicians and other clinicians while utilizing the iHealth Share Care platform and devices designed to track and monitor outcomes and provide remote vitals monitoring and educational interventions.
Provide remote support to patients in the iHealth Unified Care program through the online platform during shifts.
Work with a cross-functional clinical operation, clinical research and product team to develop and constantly improve digital health solutions for chronic disease management
Develop clinical protocols and educational content for our diabetes and hypertension self-management education program that meets AADE and AHA standards.
Teach our elderly patients on how the mobile app and medical device works, as well as helping them with troubleshooting with any tech issues. This role requires you to be tech-savvy, and you must be comfortable with working with our internal messaging (Google Hangout, Zoom, Google spreadsheet, pdf, printers, etc. ). Also requires you to be patient and able to explain technology to a non-tech savvy population.

License/Certifications
RD/RDN (Registered Dietitian/Nutritionist)
2 plus years of clinical experience required
CDCES, Lipid Specialist, Cardiovascular related experience highly preferred
Qualifications and Skills:
Excellent communication skills in patient care
Excellent trainability and flexibility
2 years or more experience working with patients with Diabetes/Hypertension/Kidney Disease/Cardiovascular Diseases in a clinical setting
Ability to perform standard clinical procedures according to established protocols
Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
Ability to maintain quality, safety, and/or infection control standards
Ability to adapt to fast-pace working environment and multi-tasking
Must be a good team player or even a team leader
Must have basic good computer skills
Excellent problem-solving and critical thinking ability
Ambitious and willing take risks and challenges
Flexible on traveling among different clinic sites and events planning in local communities (estimation is up to 20%)
