</div><div><strong>Responsibilities:</strong></div><ul>
<li>Assist patients with physical therapy exercises and rehabilitation programs.</li>
<li>Monitor patient progress and report changes to the supervising physical therapist.</li>
<li>Educate patients and families on home exercise programs and safety techniques.</li>
<li>Maintain accurate documentation of patient care and progress notes.</li>
<li>Collaborate with a multidisciplinary team to ensure comprehensive patient care.</li>
<li>Ensure compliance with health care regulations and company policies.</li>
<li>Provide compassionate support and encouragement to patients during therapy sessions.</li>
<li>Travel to patients' homes within the service area as required.</li>
</ul><div><strong>Requirements:</strong></div><ul>
<li>Active Physical Therapist Assistant (PTA) license in California.</li>
<li>Experience in a home health care setting preferred.</li>
<li>Strong communication and interpersonal skills.</li>
<li>Ability to work independently and as part of a team.</li>
<li>Knowledge of physical therapy principles and practices.</li>
<li>Current CPR and First Aid certification.</li>
<li>Reliable transportation and valid driver's license.</li>
<li>Compassionate demeanor with a patient-centered approach.</li>
