<li>Conduct thorough assessments and develop individualized care plans for wound care patients.</li>
<li>Administer wound care treatments and monitor patient progress effectively.</li>
<li>Educate patients and families on wound care management and health maintenance.</li>
<li>Collaborate with healthcare teams to ensure comprehensive patient care.</li>
<li>Document patient care activities accurately and timely in electronic health records.</li>
<li>Provide compassionate support and emotional care to patients and their families.</li>
<li>Stay updated on best practices and advancements in wound care management.</li>
<li>Participate in ongoing training and professional development opportunities.</li>

<li>Current LVN license in California with Wound Care Certification.</li>
<li>Minimum of 1-2 years of experience in home health or wound care settings.</li>
<li>Strong assessment and critical thinking skills.</li>
<li>Excellent communication and interpersonal abilities.</li>
<li>Compassionate demeanor with a patient-centered focus.</li>
<li>Ability to work independently and as part of a team.</li>
<li>Proficient in electronic health record systems.</li>
<li>Current CPR certification and knowledge of infection control practices.</li>
