• *Key Responsibilities**

As a Customer Service Representative, you'll be responsible for:

• Providing after-hours support to triage urgent clinical needs of patients, caregivers, and referral sources
• Assisting local Patient Registration teams with new patient referrals and ensuring accurate information collection
• Meeting standardized performance metrics for calls and referrals
• Communicating coordination of care information with team members, patients/caregivers, referral sources, and home health agencies
• Delivering quality customer service during all interactions and maintaining organized file systems for document tracking and patient management

• *Requirements**

To be successful in this role, you'll need:

• High school graduate or GED required
• At least one year of previous call center experience
• Proficiency with Microsoft Office Suite and keyboarding skills
• Strong interpersonal and communication skills

• Knowledge of computerized information systems
