About Foresight Health

Foresight Health is an AI-native healthcare provider building the future of chronic care. We’ve raised $8.5M in seed funding from Quiet Capital, Susa Ventures, and Neo. In our first three months, we’ve partnered with 11 neurology and endocrinology clinics across 5 states, with capacity to serve more than 5,000 patients, and we’ve built the legal infrastructure to operate compliantly in all 50 states. Our CEO, Tienlan, was sponsored by Masayoshi Son to conduct frontier health AI research at Berkeley.

The Role

In your first 1 to 3 months, you’ll be fully patient-facing:

Conduct consent calls with new patients and families

Build individualized care plans covering symptoms, modifiable risk factors, and medical and non-medical interventions

Deliver monthly check-ins: assess changes in baselines, recommend care resources, and coordinate with caregivers and clinics

Handle care logistics, including scheduling referrals, flagging medication refill needs, and sending appointment reminders

Find and vet regional and disease-specific resources (exercise, diet, sleep, social engagement) for your patients

As we grow, you’ll take on leadership:

• Train and onboard new LVNs and RNs

• Manage escalating alerts from LVNs and make clinical decisions

• Partner with the CEO and Clinical Supervisor to improve care quality and clinical protocols

Throughout all of this, you’ll collaborate with our technology team—giving feedback on our care platform and helping shape how AI fits into the care we provide.

Active RN license

CCM and/or PCM experience preferred
