Job Title: Remote Clinical Intake Nurse

Location: Remote

Job Summary:
We are seeking a detail-oriented and proactive Clinical Intake Nurse to join our team. This remote role is responsible for managing incoming referrals, coordinating with hospital partners, and supporting the development and maintenance of a strong and balanced patient census.

Key Responsibilities:

• Monitor hospital referral portals daily to identify and respond to new patient referrals in a timely manner

• Communicate effectively with hospital case managers and social workers to coordinate patient admissions

• Review clinical information to ensure appropriate patient placement and eligibility

• Support and maintain a healthy census mix aligned with organizational goals

• Collaborate with internal teams to facilitate smooth admission processes

• Maintain accurate and up-to-date documentation of all referral and intake activities

Qualifications:

• Experience with clinical intake, case management, or admissions preferred

• Strong communication and relationship-building skills

• Ability to work independently in a remote environment

• Proficiency with electronic medical records and referral portal systems

Preferred Skills:

• Experience working with hospital systems and post-acute care referrals

• Strong organizational and time management abilities

• Critical thinking and clinical assessment skills
