You'll collaborate closely with licensed nurses, case managers, and interdisciplinary clinical staff to deliver comprehensive, compassionate support. Your primary focus will be on accurate data entry, documentation, and resource coordination that powers arenaflex’s telehealth and member‑service platforms. Key Responsibilities Enter, verify, and maintain member data in arenaflex’s secure electronic health record (EHR) system with 99.9% accuracy. Conduct telephone and electronic outreach to assess member needs related to medical, financial, transportation, safety, and housing concerns. Partner with registered nurse case managers to develop individualized care plans, ensuring alignment with each member’s benefit plan. Research and connect members to community resources, including Medicaid, SNAP, housing assistance, transportation services, and mental‑health support. Document all member interactions, referrals, and follow‑up actions in compliance with HIPAA and arenaflex’s internal privacy standards. Identify patterns in member data that may indicate gaps in service delivery, and communicate insights to quality‑improvement teams. Participate in virtual team huddles, case reviews, and continuous‑learning webinars to stay current on best practices. Adhere to arenaflex’s scheduled hours: Monday‑Friday, 8:00 am – 4:30 pm Pacific Standard Time, with no weekend or holiday obligations. Essential Qualifications Active, unrestricted license as a Licensed Social Worker (LSW), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Independent Social Worker (LISW) in your state of residence. Minimum of 2 years of experience in medical social work, case management, or a closely related field. Master’s degree in Social Work (MSW) from an accredited program. Proven ability to accurately input large volumes of data while maintaining confidentiality and compliance. Strong verbal and written communication skills; comfort conducting phone interviews and electronic correspondence. Reliable high‑speed internet connection and a dedicated home office space that meets arenaflex’s privacy standards. Preferred Qualifications Experience with telehealth platforms, EHR systems (e.g., Epic, Cerner), or other healthcare IT tools. Certification in Case Management (CCM) or related credentials.
