<p>This position will be based in Ulster county, NY for the first 90 days and the schedule will be Mon-Fri, 8a-4:30p, with 1-2 Saturday shifts. </p><p><strong>After the first 90 days, this role will be FULLY REMOTE</strong> and the schedule will be either 9:30a-6p or 11:30a-8p with rotating Saturdays 8a-4:30p (every 3-6 weeks). </p><p>This position supports a healthcare call center by triaging patients and providing clinical guidance to ensure appropriate care and efficient coordination with providers and care teams. </p><p>Responsibilities </p><ul><li><p>Triage patient calls to assess medical needs and recommend appropriate levels of care </p></li></ul><ul><li><p>Coordinate with physicians and care teams to support patient care and follow-up </p></li></ul><ul><li><p>Schedule appointments, process medication refills, and submit prior authorizations </p></li></ul><ul><li><p>Document patient interactions accurately in the electronic health record (EHR) </p></li></ul><ul><li><p>Conduct outreach to patients and pharmacies to support care coordination and medication safety </p></li></ul><p>Requirements </p><ul><li><p>AAS in Nursing required; BSN preferred </p></li></ul><ul><li><p>Active New York State RN license with BCLS, Infection Control, and Child Abuse certifications </p></li></ul><ul><li><p>3+ years of RN experience in ambulatory, medical/surgical, ER, or physician office settings </p></li></ul><ul><li><p>Experience with telephone triage and familiarity with EHR systems (EPIC preferred) </p></li></ul><ul><li><p>Strong communication, clinical judgment, and computer skills; bilingual Spanish preferred </p></li></ul><p>Compensation $76,000 - $82,000<br><br><span><span><span><br></span><span><br></span><span> </span></span></span></p>
