Job Description:
• Perform Medication Therapy Management (MTM) interventions
• Complete medication reconciliation and identify optimization opportunities
• Provide patient-centered education on medications and disease management
• Support chronic care initiatives (e.g., diabetes, hypertension, COPD, CHF)
• Use behavioral techniques such as motivational interviewing to improve adherence
• Collaborate with providers on treatment planning and medication adjustments
• Detect and resolve medication-related risks and discrepancies
• Ensure timely and accurate documentation of all encounters
• Facilitate smooth care transitions and ongoing care coordination
Requirements:
• Doctor of Pharmacy (PharmD)
• Active pharmacist license in at least one of the following states: CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT
• Open to obtaining additional licensure as required
• Board certification (e.g., BCPS, BCGP) preferred
• Experience in: MTM delivery, Ambulatory or outpatient care, Chronic disease management programs, Patient engagement and education
• Familiarity with programs such as CCM, TCM, RPM is beneficial
