Diabetes Care Coordinator

 The ideal candidate will work with the Endocrinology and OB teams to support the care of pregnant patients with diabetes, and assist with tracking, monitoring, outreach, and supporting all patients involved in this program. 

 

Essential Functions  

 

Requirements:  

 

 

Schedule: Monday-Friday, 9AM-5PM with options for some remote access 

 

Working Conditions and Physical Effort: 

 

Manual Skills Significant (more than 50%) portions of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity. 

Physical Effort: Some (less than 50%) portions of daily assignments involve prolonged standing,  transporting patients, material, and equipment, or lifting, moving, or carrying heavy (over 30 lbs.) materials for intermittent periods throughout the day. 

Physical Environment: Some (less than 50%) portions of daily assignments involve exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes.  Working surfaces may be unlevel, slippery, or unstable. 

Work Schedule: Work may require evening, night, holiday, or weekend assignments on occasion. 

Occupational Risks: Some (less than 50%) portions of daily assignments involve occupational risk, such as cuts, burns, exposure to toxic chemicals, injuries from falls, or back injury sustained with assisting in moving/lifting/positioning patients, equipment, or materials. 

Bloodborne Pathogens Exposure: OSHA Category III - Exposure not anticipated. Work does not involve exposure to blood, body fluids, or tissue, and does not require performing unplanned exposure tasks. 

Safety Equipment: Job does not require the use of safety equipment, such as HEPA masks, for prevention of infection. 

Ergonomics Risks:  Significant portions (more than 50%) of daily assignments involve ergonomic risk. 

 Salary - $75,000
